Missed Appointment Fees

Missed appointment fees apply for consultation, feedback, and evaluation appointments. Please refer the Informed Consent document for details. 

Evaluation and Consultation Fees

The hourly fee for a comprehensive evaluation includes all time spent, to include parent and child interviews, testing, scoring, report writing and interpretation, recommendations, and feedback session. Additional fees for consultation apply. 

Payment

Accepted forms of payment include cash, check, and credit card.  Payment for services is due at each visit unless other arrangements have been made in advance.

Information for patient submission to health insurance is provided for the beneficiary to submit to their insurance company. It is recommended that you check with your insurance company regarding coverage for neuropsychological diagnostic evaluation services under the procedure code of 96118. The diagnosis code is determined at the end of the evaluation.

Written reports are provided after the balance is paid in full.

Security of Test Materials

Dr. Reed strives to maintain the integrity and security of test materials and other assessment techniques consistent with law and contractual obligations, and in a manner that permits adherence to the ethics of psychologists. While test scores and data are provided, test forms are not provided; and observation or videotaping of testing is not allowed. The Health Insurance Portability and Accountability Act (HIPAA) does not supersede federal copyright law or access to psychotherapy records in all instances.

Copies of Records

Parents are provided with two copies of the comprehensive evaluation report; and a copy will be forwarded to the referral source if desired. The remainder of the file will be electronically archived and secured. Electronic files will not be sent via electronic mail due to security issues. Additional copies of information and/or reports will be provided to legal guardians upon request and with appropriate written Authorization to Release Confidential Information (see Forms). Copies of raw data test material will be copied and/or forwarded to another neuropsychologist with appropriate written Authorization to Release Confidential Information.  Fees are charged for handling and photocopying in accordance with the Michigan Medical Records Act.
